Criteria to apply: i) Applicants must FIRST apply to (and receive a response from) AT LEAST ONE of the above-mentioned youth in sport grant opportunities (which require NOA and/or Child Tax Benefit information). ii) Applicant must be registered with Brock Minor Hockey for the current season iii) Applicant must live within the Brock Minor Hockey boundaries iv) Applicant must be willing to provide a non-identifying description of the financial barriers this player/family faces (*real names will only be disclosed to the BMHA president, treasurer and registrar, and will be kept absolutely confidential from donors, the BMHA executive, and the public) v) If the applicant meets the above criteria, please proceed with your application by completing this form
