Use this form to submit your Fundraising and Sponsorship intentions for the season. This form is to be submitted along with the Team Budget according to the Brock Minor Hockey Manual of Operations sections 9.2 and 10.3 which outline Team Fundraising and Team Sponsorship expectations. No Fundraising or Sponsorship Activities can commence until you have received both confirmation of receipt, and approval to proceed with only the items outlined on this application. If your plans or budget change, please re-submit the form. Team Budget Submission is MANDATORY, and shall only be submitted via this Online Form.

I agree to the terms of running the team fundraiser(s) and/or team sponsorship(s) as set out by the fundraising/sponsorship section of the Manual of Operations sent to you by the Fundraising manager. I also agree to submit the provided budget spreadsheet along with the submission of this form with all projected season spending, as well as updates by January 1st, 2024 and April 1st, 2024 accounting for all team funds. Our team will not begin fundraising activities, or collection of sponsorship funds without receiving confirmation of approval to proceed from this application.
